  • full name, home and business street address and business telephone number of each owner, officer, director, principal and person with an ownership interest of 10 percent or more in the business and the percentage of ownership held
  • If an owner holds a professional or occupational license in the state, provide the license type, agency, number, and date issued
• Certification form
• Certificate of insurance with the company listed as the insured
  • Policy of commercial general liability insurance in a minimum amount of $500,000 per occurrence
  • Mmust include an effective policy number, the date of issuance, the effective date and your signature
• If prior misconduct occurred, provide proof of rehabilitation
  • the nature and responsibility of the position the convicted individual would hold;
    • the nature and seriousness of the offense;
    • the circumstances under which the offense occurred;
    • the date of the offense;
    • the age of the individual when the offense was committed;
    • whether the offense was an isolated or repeated incident;
    • any social conditions which may have contributed to the offense;
    • any evidence of rehabilitation including, among other things, good conduct in prison or in the community, counseling, or psychiatric treatment received.

• If you employ salespersons to execute home repair contracts with homeowners on behalf of your company, you must license each individual so employed. Officers of the corporation are not required to be licensed as a home repair salesperson

• Signatures for this application are required by the most senior person in the company
  • Corporation - President and secretary
  • LLC - managing member and witness
  • Partnership - partner and witness
  • Sole Proprietorship - sole proprietor and witness
• The corporate seal or a copy of the corporate seal must be affixed in the case of a corporate applicant
• Signatures must be witnessed by anotary public or attorney
• An in-state New Jersey office location is required

• Certified copy of the company's articles of incorporation or formation
• Copy of your New Jersey authorization to do business (foreign qualification) if not registered domestically in the state
• A copy of any DBA's, alternate, or fictitious names registered to be used in New Jersey. All DBA's must be registered
• Personal Certifications must be completed by all officers, directors, substantial stockholders (10% or more), members, partners and/or owners
• A financial statement as prepared by your accountant/office manager/bookkeeper demonstrating net worth
  • In the instance of a newly formed company, attach a start-up balance sheet
• Attach a copy of deed, lease or rental agreement for location to be licensed as listed on the application
• Attach two (2) photographs of the office location
  • one exterior and one interior
  • photograph must clearly show the required business sign
• If you conduct business from a residence, you must provide proof
  • Attach copies of letters you have sent to your local post office and telephone company advising them of the use of your residence for your business
  • A copy of the deed, lease, or rental agreement for the residence.
• Certificate of Insurance showing that you have public liability and worker’s compensation insurance
  • The certificate should show the New Jersey Department of Banking and Insurance at the address on the top of the application as a party of interest
  • The certificate should also show your agent’s name, address and telephone number
• Submit proof of Home Improvement Contractor registration with the New Jersey Division of Consumer Affairs

Home Builder Registration Application

Applications may be deemed incomplete and closed after 30 days, if after having been notified by the New Home Builder Registration Unit of a deficiency, the applicant fails to correct the deficient application submission. If an application is closed a new application and a new $200.00 application fee is required to restart or continue the application process.

• The name of the business or person who owns the property and is transferring title is the entity/person that must register as a new home builder and issue the warranty for the new home. This exact name should appear on your application for registration as a new home builder in the state of new jersey. If you are the only owner of the business and are doing business under your own name as a sole proprietor, list your own name.
• List all other names under which the new home builder will be doing business (include a copy of the Trade Certificate or Alternate Name
  Certificate of assumed name on file with the county or state.)
  • D/B/As, AKAs, trade names, fictitious names, etc. must be filed with the State Department of Treasury or the county or town where the new home builder is conducting business.

• Applicants must provide Business Formation documents:
  • Certificate of Incorporation
  • Partnership Agreement
  • Joint Venture Agreement
  • Certificate of Formation
  • For LLC: Trust Agreement
• If the Certificate of Incorporation or Certificate of Formation is more than 1 year old, also submit the Business Entity Status Report, Annual Report, or Long Form Standing showing the current principals/officers and status of the company.
• Out of state (foreign) entities must also submit a Certificate of Authority to do Business in New Jersey.

Home Builder Registration Application (Renewal)

• Submit the Business Entity Status Report, Annual Report, or Long Form Standing showing the current principals/officers and status of the company
• If there have been any changes since the previous renewal with regard to principals, members, directors, partners or officers in this building entity, please include documentation reflecting those changes
  • For Corporations provide-a copy of the annual report status form indicating the current principals in the entity. A copy of a unanimous consent in writing or an action by consent in writing from the secretary of the entity indicating appointments/regenerations and/or terminations of officers is acceptable.
  • For Limited Liablity Companies provide a copy of the operating agreement or annual report stylus form indicting the current principals in the company.
• Provide Operating Agreement (if available).
• Provide judgment and discharge documents, if there were judgments issued against the building entity.
• Provide applicable documents, if the building entity has gone through a bankruptcy, business reorganization, fianancial reorganization or liquidation.
• If you will be using a private new home warranty plan, provide private New Home Warranty acceptance letter and member enrollment number, or proof of current membership enrollment. (If your company is not yet enrolled, once enrolled you have 30 days to submit proof of enrollment
  to the Bureau.).
