Brown County, Ind., Building Contractor Ordinance 12-17-90A

• Liability Insurance in an amount of no less than $100,000 per person and $300,000 per occurrence.
• Worker's Compensation Insurance

Electrical Contractors are required to take and pass an exam with a 75% or higher. Exams are administered by the Brown County Building Department. The fee for examination is $35.

A contractor must be licensed in the City of Indianapolis before being approved for a City of Beech Grove license.

An active certificate of insurance should be on file with the City of Crawfordsville.

A recorded Lake County (IN) Bond in the amount of $5,000

Liability insurance in an amount of no less than $500,000

• Certificate of Insurance submitted required to be original form to include policy numbers, and stipulating “City of East Chicago, Indiana, 4525 Indianapolis Blvd., East Chicago, IN” as Certificate Holder. 
• Workmen’s compensation covering all employees on any work done within the City of East Chicago.
• An independent contractor not including Workmen’s Compensation on insurance policy is required to apply for an
  “Independent Contractor Affidavit of Exemption” from the Indiana State Workmen’s Compensation Board (800-824-2667). Form is available at the Building Department office.
• Public liability insurance in the amount of not less than $100,000 for one person or $300,000 for one accident.
• Property damage insurance in the amount of not less than $50,000.
• Such workmen’s compensation, public liability and property damage insurance shall be kept in full force and effect during the entire licensed period and certificates to that effect annually.

• Surety bond in the sum of $5,000 payable to the City is required to be recorded with the: Lake County Recorder’s Office 2293 N. Main Street, Crown Point, IN 46307 (219) 755-3730.
• Bond accepted stipulating “All Cities, Towns and Municipalities in Lake County, IN.”
• Such bonds shall be resubmitted each year before renewal of the license is granted.

For foreign and/or out of state contractors – Certificate of Authority required to be submitted from State of Indiana Office of the Secretary of State.

All businesses maintaining and operating its offices in the City of East Chicago are required to obtain and hold a valid business license from the City of East Chicago prior to receiving a contractor’s license from this department. General Liability Insurance (no minimum)

• For Masters License, Commercial General Contractor’s License or an Unlimited Roofing License, an applicant shall, before being granted a license, make, execute and deliver to the Building Commission proof of general liability insurance in an amount not less than One Million Dollars ($1,000,000.00), with good and sufficient surety insuring all users of the licenses and indemnifying and holding harmless those persons sustaining loss or damages resulting from any and all work done under that license.
• Residential General Contractor, Limited Roofing Contractor, Wrecking Contractor or a Sign Erector Class B, shall before being granted a license, make, execute and deliver to the Building Commission proof of general liability insurance in an amount not less than Five Hundred Thousand Dollars ($500,000.00), with good and sufficient surety insuring all users of the licenses and indemnifying and holding harmless those persons sustaining loss or damages resulting from any and all work done under that license.
• For all other Sub-Contractor Licenses, an applicant shall, before being granted a license, make, execute and deliver to the Building Commission, proof of general liability insurance in an amount not less than Three Hundred Thousand Dollars ($300,000.00), with good and sufficient surety insuring all users of the licenses and indemnifying and holding harmless those persons sustaining loss or damages resulting from any and all work done under that license.
• Each applicant for a Master’s or Contractor or Sub-Contractor License, shall, before being granted his/her license, provide proof of Worker’s Compensation coverage or an affidavit stating that by Indiana Law he/she is not required to have Worker’s Compensation Insurance.
  • This must be done within six (6) months of passing the exam and approval by the Licensing and Disciplinary Board.

• Each applicant for a Master’s or Contractor or Sub-Contractor License, shall, before being granted his/her license, make, execute and deliver to the Building Commission a Surety Bond in the amount of Twenty Five Thousand Dollars ($25,000.00) established in the applicant’s name on a form provided to the insurance company by the Building Commission. 

• General Liability Insurance Certificate is required. 
  • Must have a minimum of $500,000 for each occurence of death or bodily injury. 
  • Must have $100,000 for each occurence of prperty damage, or must have combined single limit coverage which covers both bodily injury and property damage, minimum $500,000 per occurence.

Liability and Property Damage Insurance in an amount not less than $100,000

• General: $5,000
• Asbestos: $10,000
• Tank Contractor: $10,000
• Surety Bond should be worded as: Board of Commissioners of the County of Lake, State of Indiana and all other Cities, Towns and Municipalities Therein

The City of Gary facilitates Contractor Licensing Exams for both general and trade contractors. There is a $150 per examination fee. Please see the Contractor's Licensing Exam Packet for more information about exam registration and scheduling. 

• A Certificate of Insurance with the City of Hammond, 5925 Calumet Avenue, Hammond IN 46320 as Certificate Holder, with liability in the amount of 100,000/300,000 bodily injury and 50,000-property damage. Landlord License holders will be required to provide proof of insurance for each property owned.
• Workman’s Compensation or a Certification of Waiver.

A $5000 bond in the name of: Board of Lake County Commissioners, all Cities, Towns and Municipalities of Lake County, Indiana. The bond must be recorded at the Lake County Recorder’s Office, located at 2293 North Main St, Crown Point Indiana 46307 in order to be accepted.

• Certificate of Insurance with the following:
  • General Liability: each occurence = minimum of 500,000
  • General Liability: general aggregate = minimum of $1,000,000
  • Workers Compensation & Employer's Liability: EA accident = minimum of $100,000
  • Workers Compensation & Employer's Liability: disease EA employee = $100,000
  • Workers Compensation & Employer's Liability: disease policy limit = $500,000
• If you are the sole employee you can apply to the state of Indianna Department of Revenue for the worker's compensation waiver. 

Minimum of $5,000 recorded with the Lake County, Indiana Office of the Recorder. The following verbage must be included on the bond in its entirety, "The board of commissioners of the county of Lake, state of INdianna, and all cities, towns and municpalities within". 

The Contractor's company information must match exactly on both the Certificate of Insurance and the Lake County Bond.

• Worker's Compensation Insurance
• The certificate holder must be listed as the City of Knox

• A bodily insurance and property damage polocy insuring the contractor in the amouint of $5000,000. 
• Workman's Compensation Insurance 

Mishawaka, Ind., Municipal Code § 101-20 — 101-292.

Original surety or permit bond in the amount of $5,000 payable to the City of Mishawaka.

The City of Muncie requires a $500,000.00 certificate of insurance and a $5,000.00 bond. Insurance and bond has to have the City of Muncie listed as the holder.

General Liability Insurance in an amount not less than $1,000,000

No less than $300,000 commercial liability coverage for each occurrence

• The City of New Castle license board administers the written and practical exams for license applicants (or sees that they are given) for each trade an applicant wishes to be licensed in
• Plumbers are licensed by the State of Indiana, but need to register to perform work within the city limits

Certificate of Liability Insurance is required. The certificate holder must list the City of Peru

City of Portage IN Municipal Code - Contractors 

• Each applicant shall also submit a certificate of insurance to the building commissioner naming the City of Portage as the certificate holder and naming the City of Portage, its officers, agents and employees as additional insureds in the minimum amounts as hereafter provided:
  • Commercial general liability of $1,000,000.00 per occurrence and $1,000,000.00 aggregate.
  • Workers' compensation as required by the State of Indiana.
• If you stated that you do not have workman's compensation insurance, you must provide a copy of your state of Indiana exemption certificate and sign and date: I hereby certify I have no employees but myself, and in the event I employ more than anyone other than myself, I will comply with city of portage municipal code section 18-59 pertaining to workman's compensation insurance.

Each applicant shall submit a copy of a current Porter County Unified License Bond in the amount of $5,000.00 with the obligee identified as "Porter County and all cities, towns, and municipalities in Porter County."

Each sewer contractor shall also provide a bond in the amount of $50,000.00 with the obligee identified as "the City of Portage."

• A certificate of liability insurance that is no less than $500,000
• Worker's compensation insurance must be included on the certificate. The certificate holder should be:

A recorded Porter County bond in the amount of $5,000

Certificate of Liability Insurance in an amount of no less than $500,000. DeKalb County Government must be listed as the certificate holder. 

A certificate of general liability and worker's compensation insurance of no less than $1,000,000.

• For residential contractors the bond amount is $5,000 per trade
• For commercial contractors the bond amount is $20,000 per trade
• The obligee on bonds must be Delaware County Commissioners

Workers Compensation certificate is needed if the contractor has more than one employee when applying for registration.

• $500,000 for combined bodily injury and property damage coverage
• $500,000 for any occurrence relative to which there is an injury or death to one or more persons
• $100,000 for any occurrence relative to where there is damage to property
• Worker's compensation insurance covering the personnel employed for death or injury arising out of the operations being performed by or on behalf of the listed contractor

The city of Jeffersonville requires contractors to hold liability insurance of $100,000/$300,000 with the city of Jeffersonville as certificate holder.

• $20,000 surety bond payable to Johnson County and/or an unknown third party. 
• The bond must be for at least a year. If a contractor wishes to be listed for 2 or 5 years then the bond must be for that duration. 

A Certificate of Insurance with the City of Marion Building Department listed as the certificate holder with limits of at least $500,000 on injury or death; and $100,000 for damage to property.

A Certificate of Insurance with the City of Marion Building Department listed as the certificate holder with limits of at least $500,000 on injury or death; and $100,000 for damage to property.

A Certificate of Insurance with the City of Marion Building Department listed as the certificate holder with limits of at least $500,000 on injury or death; and $100,000 for damage to property.

• Liability Insurance in an amount no less than $500,000
• Worker's Compensation Insurance or a copy of Affidavit of Independent Contractor Certification which you have received from the State of Indiana

$20,000 License and Permit Bond payable to Marshall County

For "Type A" General Contractors:

• $500,000 one person and $1,000,000 one accident public liability insurance
• $250,000 property damage insurance
• Workman's Compensation coverage as required by the state of Indiana

For all other contractors:

• $100,000 one person and $300,000 one accident public liability insurance
• $50,000 property damage insurance
• Workman's Compensation coverage as required by the state of Indiana

• Public liability insurance in the amount not less than $300,000 in the case of damage or injury to 1 person and not less than $500,000 in case of damage or injury to more than 1 person

• $500,000 bodily injury and property damage combined single unit
• Must include worker's compensation insurance

$5000 with obligee and certificate holder as Pulaski County Building Department

Insurance and bond shall be issued by an insurance company authorized to do business in the State of Indiana

Proof of Liability Insurance – insuring the listed contractor and naming the Director of County Commissioners of
Putnam County or an unknowing third party as additional insured

Surety Bond in the amount of at least $20,000 payable to Putnam County or an unknown third party as oblige

A certificate of insurance of at least $500,000 of general liability and bodily injury insurance, naming the Town of St. John an additional insured.

Lake County Bond of $5,000

• Public Liability and Property Damage insurance not less than $1,000,000
• Worker's Compensation coverage as required by the State of Indiana

$5000 and must read as follows: Payable to Starke County, All Towns, Cities, and Municipalities therein

All insurance and bonds shall be issued by an insurance company authorized to do business in the State of Indiana

• Liability Insurance in an amount of no less than $500,000
• Worker's Compensation Insurance or a waiver from the State of Indiana

Any individual who engages in or who supervises the work of commercial or residential construction shall be required to be registered with the Building Department

All building contractors shall furnish, as part of this application: (a) a valid, current Certificate of Insurance showing compliance with Indiana Worker’s compensation and occupational disease laws, or a current worker’s compensation clearance certificate; and (b) a Certificate of Insurance demonstrating minimum combined single limits of five hundred thousand dollars per occurrence for bodily injury and property damage.

A certificate of insurance in the amount greater than or equal to $500,000.00 for single limit coverage for bodily injury and property damage. The certificate of insurance must show the Town of Dyer as a certificate holder.

A current $5,000.00 bond, which must be recorded in Crown Point at the Lake County Government Center located at 2293 N. Main Street (219) 775-3730. If a copy of the original bond displaying the County recorder's stamp is on file with the building department, the continuation certificate for that same bond number does not need to display the recorder's stamp.

• General Liability Insurance: $100,000-$300,000
• Vehicle Liability Insurance: $100,000-$300,000
• Property Damage Insurance: $100,000
• Worker's Compensation Certificate or an exemption from Indiana Department of Revenue

The certificate holder must be the Town of Griffith.

Certificate of insurance showing property damage of not less than $50,000 and liability isnuranc eof not less than $100,000, with the city as a certifcate holder.

$5,000 Surety Bond

• Certificate of insurance with the certificate holder listed as : Town of Munster, 1005 Ridge Road, Munster, IN 46321
• Public Liability: shall be an all-perils or umbrella policy of at least $1,000,000
• Workers Compensation: workers compensation insurance if applicable. If not applicable, an exemption form from the State of Indiana Workman's Compensation Board must be obtained. The form may be obtained on the in.gov/wcb/website (Form 45889) or a complimentary copy of the application may be requested from our office.

One of the options below:

• Copy of recorded $5,000 Bond made out to Lake County and all Cities and Towns therein; bond must be recorded at theLake County Recorder’s Office, 2293 North Main Street, Crown Point, IN 46307, (219) 755-3730
• Original $5,000 Bond made out to Town of Munster, this must be mailed or dropped off in one of our drop boxes

Prior to the issuance of any license as a general building contractor or subcontractor, the town shall conduct a limited criminal background check pertaining to the applicant.

• General Liability: $1,000,000
• Per Occurrence: $500,000
• Worker's Compensation: $100,000 for each accident + $100,000 disease-per employee + $500,000 disease policy limit
• The Town of Schererville must be the certificate holder
• If working on Town-owned property, the Town of Schererville must be listed as additional insured
• If no employees, the Town of Schererville requires a clearance certificate from the worker's compensation board of Indiana

• $5000 and the obligee should read Board of Commissioners of Lake County, IN and all cities, towns and municipalities in Lake County, IN. 
• The bond must be recorded at the Lake County Recorders' Office located at 2293 N. Main Street Crown Point, IN 46307

Electricians must pass the Town of Schererville electrical exam. There is no fee for the test.

Liability insurance for each occurrence in the amount of $100,000 to $300,000 by an insurance company licensed to do business in the State of Indiana. The Town of Sellersburg must be listed as the certificate holder.
