  • Certificate of Insurance submitted required to be original form to include policy numbers, and stipulating “City of East Chicago, Indiana, 4525 Indianapolis Blvd., East Chicago, IN” as Certificate Holder. 
  • Workmen’s compensation covering all employees on any work done within the City of East Chicago.
  • An independent contractor not including Workmen’s Compensation on insurance policy is required to apply for an
    “Independent Contractor Affidavit of Exemption” from the Indiana State Workmen’s Compensation Board (800-824-2667). Form is available at the Building Department office.
  • Public liability insurance in the amount of not less than $100,000 for one person or $300,000 for one accident.
  • Property damage insurance in the amount of not less than $50,000.
  • Such workmen’s compensation, public liability and property damage insurance shall be kept in full force and effect during the entire licensed period and certificates to that effect annually.
Bond Requirements:
  • Surety bond in the sum of $5,000 payable to the City is required to be recorded with the: Lake County Recorder’s Office 2293 N. Main Street, Crown Point, IN 46307 (219) 755-3730.
  • Bond accepted stipulating “All Cities, Towns and Municipalities in Lake County, IN.”
  • Such bonds shall be resubmitted each year before renewal of the license is granted.

City of Evansville/Vanderburgh County General Contractor License

Insurance Requirements:
  • For Masters License, Commercial General Contractor’s License or an Unlimited Roofing License, an applicant shall, before being granted a license, make, execute and deliver to the Building Commission proof of general liability insurance in an amount not less than One Million Dollars ($1,000,000.00), with good and sufficient surety insuring all users of the licenses and indemnifying and holding harmless those persons sustaining loss or damages resulting from any and all work done under that license.
  • Residential General Contractor, Limited Roofing Contractor, Wrecking Contractor or a Sign Erector Class B, shall before being granted a license, make, execute and deliver to the Building Commission proof of general liability insurance in an amount not less than Five Hundred Thousand Dollars ($500,000.00), with good and sufficient surety insuring all users of the licenses and indemnifying and holding harmless those persons sustaining loss or damages resulting from any and all work done under that license.
  • For all other Sub-Contractor Licenses, an applicant shall, before being granted a license, make, execute and deliver to the Building Commission, proof of general liability insurance in an amount not less than Three Hundred Thousand Dollars ($300,000.00), with good and sufficient surety insuring all users of the licenses and indemnifying and holding harmless those persons sustaining loss or damages resulting from any and all work done under that license.
  • Each applicant for a Master’s or Contractor or Sub-Contractor License, shall, before being granted his/her license, provide proof of Worker’s Compensation coverage or an affidavit stating that by Indiana Law he/she is not required to have Worker’s Compensation Insurance.
    • This must be done within six (6) months of passing the exam and approval by the Licensing and Disciplinary Board.

  • Each applicant for a Master’s or Contractor or Sub-Contractor License, shall, before being granted his/her license, make, execute and deliver to the Building Commission a Surety Bond in the amount of Twenty Five Thousand Dollars ($25,000.00) established in the applicant’s name on a form provided to the insurance company by the Building Commission.

How to Apply:
  1. Complete the application in ink or type
    1. Make sure you clearly state the license for which you wish to test.
    2. Obtain three (3) letters of recommendation from reputable business or professional men, not related by blood or marriage, vouching for the applicant’s reputation as to honesty, integrity, and good character.
    3. Fill out the middle of the Criminal Record History form and have it notoarized.
    4. Electrical and HVAC recommendation letters must be on letterhead and notarized that prove at least four (4) years of experience in the field for which you are applying.
    5. The application and letters must be submitted at one time to the Building Commission.
  2. The applications will be submitted to the proper boards for approval to take the test.  Once this is done, you will be contacted (via e-mail or phone) with approval to contact Ivy Tech (812)429-9852 to schedule and pay required fee for test you are taking.
  3. Exam scores are reviewed by the Licensing and Disciplinary Board on the fourth Thursday of the month for acceptance or denial of license.
  4. The applicant will be contacted by the Building Commission for the completion of the licensing process, which is the proper fee and providing proof of the required insurance and surety bond.

Renewals not received by January 31st, of each year will be subject to a late fee of $100 ($50 late fee for Plumbing Contractors). Licenses that are not renewed by June 30th of each year will be void.

    registration must be submitted with this application. If the county does not record DBA’s the contractor should submit a letter stating the county policy.

    • Must indicate the policy number or “Binder” (a binder is only valid for 30 days from date of issuance)
    • Must identify the effective and expiration dates of the coverage
    • Must name the sole proprietor, partnership, or corporation as the insured
    • Must indicate coverage for “General Contractor”\
    • Must not limit coverage to a single job or address or to only when work has been permitted
    • Must name the “Consolidated City of Indianapolis” as Additional Insured. The additional insured statement must be shown on the front/face of the Certificate. If an endorsement is needed, use ISO CG2036 0413 or an equivalent.
      o If an endorsement is listed on the certificate, a copy of the endorsement must be provided.
    • Must not require a written contract or agreement for additional insured
    • Must list the City of Indianapolis as Certificate Holder with Department’s address
    • Must notify the Department, in writing, at least 15 days prior to cancellation
  • WORKMAN’S COMPENSATION INSURANCE*
    • Must carry Workman’s Compensation Insurance for workers employed in Indianapolis/Marion County
    • Must identify the effective and expiration dates of coverage
    • Must indicate the policy number or “Binder” (a binder is only valid for 30 days from date of issuance)

      *If you do NOT have employees:
      LLCs, Partnerships, and Sole Proprietors: Must complete the Workman’s Compensation waiver portion on the business license application
      Corporations: Due to the nature of a corporation, the owner/principal becomes an employee of the corporation (including Sub-S Corporations). By Indiana State Law, they are required to carry workman’s compensation coverage – at least covering the owner/principal.
