  • List the top four suppliers the company has been associated with in regards to pharmaceutical products that were sold, dispensed or distributed within the last year. 
  • Provide a copy of your NAMP Drug Distributor or VAWD certification (if applicable)
    • If the company is not accredited by NABP and/or FDA registered, they must: 
  • Provide a copy of your FDA registration (if applicable)
  • Provide a list of all officers, directors, shareholders (if non-publicly traded), and/or partners of the company and their percentage of ownership, if any.
  • Provide a list of the top four corporation shareholders and their percentage of ownership (only if publicly traded)
