Washington Charitable Gift Annuity Registration

3 years of continuous existence immediately preceeding licensure.

Minimum Unrestricted Cash or Cash Equivalent Requirement:

Audited financial statements are required within 9 months after the organization's fiscal year end.

Notes:

In addition to filing the annual report, charities must also submit a public inspection copy of their IRS form 990 within 15 days of its filing with the IRS.

Washington Health Care Benefit Manager Registration

How to Apply:
  1. Fill out the Account Creation and Request for Application form. A confirmation email will be sent to confirm your request.
  2. Once the request is approved, you'll be emailed instructions for paying the $200 registration fee.
  3. After you pay the fee, you'll be sent a receipt and links to the HCBM application form and DocuSign portal where you’ll upload your application.
  4. Upload the completed application and all supporting documents to the DocuSign portal. Sign the application and choose “Finish” to submit your application.
Required Attachments:
  • The identity of the health care benefit manager and of persons with any ownership or controlling interest in the applicant including relevant business licenses and tax identification numbers, and the identity of any entity that the health care benefit manager has a controlling interest in.
  • A copy of the health care benefit manager's certificate of registration with the Washington State Secretary of State

Agency Fee:

The renewal fee varies from year to year. The minimum fee is $500 and may increase based on a proportional share of each health care benefit manager's Washington state annual gross income as reported to the insurance commissioner.

Due:

Registrations expire on June 30 of each year, but renewals must be submitted by March 1.

How to Apply:
  1. By March 1, log into the Filing and Payment Center to access your annual gross income report.
  2. Use the annual gross income report to submit your state gross business income and supporting documents.
  3. By June 1, you'll be sent an invoice for the renewal fee. You can also find it in the Filing and Payment Center.
  4. By July 15, pay your renewal fee online or mail it with a printed invoice that includes your WAOIC number.
