The requirements are as follows: Policies issued outside the State of Florida must identify proof of coverage for the State of Florida in the "Description of Operations" box. The company and individual names listed on the certificate must match the corresponding State Licenses. License numbers of individuals qualifying a company must be listed in the "Description of Operations" box of the certificate.
