$24 for 1-3 employees
$8 per employee for 4-20 employees
$160 + $6 each in excess of 20 for 21-100 employees
$640 plus $6 each in excess of 100 for 100 or more employees

Required Attachments:

A notarized SAVE Affidavit for citizenship verification
A notarized E-Verify Affidavit for exempt or non-exempt employer status as required by the State of Georgia.
If incorporated or forming an LLC, include your articles of incorporation.
