$125 application fee + $65 card fee (once application is approved)

• 2x2 passport-style photo of applicant
• Copy of City or County competency card (if applying via reciprocity)
• Letter of Reciprocity for exams (if applicable)
• 2 letters of recommendation vouching for applicant's reputation as to honesty, integrity, and good character
  • Recommendations from relatives are not accepted
  • Letters must be on letterhead or notarized
• Proof of fictitious name registration
• Credit Reports
  • All reports must be submitted directly to Contractor Licensing in a sealed envelope and must include a public records search
  • Letter of recommendation will be required for all negative items on the report even if resolved
  • Supporting documentation includes: applicants credit report, corporate/company credit report
  • Partner's credit report

Renewals are due on the same date as the associated state-level license renewal
