Required Attachments:
  • Provide a copy of a State-issued license for all professionals working in the
    business location (doctor, lawyer, veterinarian, massage therapist, etc).
  • If home-based, provide proof of residency (utility bill in applicant’s name or driver’s license).
  • Complete (2) Notarized affidavits:
    • Affidavit of Presence in the United States
    • Private Employer Affidavit Pursuant to O.C.G.A 36-60-6(d)
