• List of officers, members, or partners including title, date of birth, and address
• A copy of the state-issued driver’s license or state-issued photo ID for the individual signing the application
• Supervisor Statement of Authority (If the supervisor certificate holder is not a company owner, officer, partner, member, or sole proprietor, the supervisor must also submit a letter on company letterhead stating they will be the “Supervisor of Record” for the company for this license type. The letter must be signed by an owner, officer, partner, member, or the sole proprietor.)
• Articles of Incorporation/Articles of Organization (for domestic entities) or Statement of foreign authority (for foreign entities)
• If doing business under a trade name (DBA), a validated copy of trade name
• A notarized copy of minutes of a meeting or other notarized documentation on company letterhead showing a complete list of company officers/members/partners
• If the entity has been registered with the state for 1 year or more, submit a current periodic report
• Sole proprietors using a trade name and individuals using a personal name must fill out and notarize the Affidavit of Lawful Presence and provide a notarized copy of your “Secure and Verifiable Identification” as listed on the affidavit.

Domestic Applicants:

1. Register your business entity with the Colorado Secretary of State.
2. Have an employee obtain a Supervisor Certificate by passing an exam and meeting experience requirements.
3. Submit a license application to the Community Planning and Development Department.

Foreign Applicants:

1. Foreign qualify your business with the Colorado Secretary of State.
2. Have an employee obtain a Supervisor Certificate by passing an exam and meeting experience requirements.
