Asbestos Individual Application Form

$99 application fee + $3 online filing fee

• Proof of successfully passing the Department’s examination for Asbestos Air Monitoring Technicians
• Proof of performing Air Monitoring as an Apprentice for 30 days of work under the direct supervision of a licensed Air Monitoring Technician working for a licensed Laboratory, Contractor, or Consultant
• 1-inch by 1-inch color photo of the applicant with a white background
• Completed DSHS “Physician’s Written Statement” signed by a doctor within the last 12 months
• An Initial Asbestos Air Monitoring Technician training certificate and training ID card (if applicable)
• Any Asbestos Air Monitoring Technician Refresher training certificates and training IDs (if applicable) obtained before submission of application

  • Asbestos Air Monitoring Technician

  • Asbestos Management planner
• Any Refresher training certificates and ID cards (if applicable) for each of the following courses obtained before submission of application:
  • Asbestos Project Designer
  • Asbestos Air Monitoring Technician
  • Asbestos Inspector
  • Asbestos Management planner
• Proof of professional liability insurance coverage in the amount $1 million for errors & omissions if working for hire, or be covered under the consultant’s employer’s policy, as specified in 295.40 of the Texas Asbestos Health Protection Rules (TAHPR).  If working for a Government agency, Provide proof of Self Insurance.
• Verifiable Evidence of one of the following:
  
  • A current license in the State of Texas as an Architect or Professional Engineer
  • Possession of a bachelor’s degree in Architecture, Engineering, Physical or Natural Science from an accredited four-year college or university and four years of experience in areas affecting Environmental or Occupational health matters.
  • Current highest full-qualification membership in a National Professional organization devoted to technical proficiency in Environmental or Occupational Health Protection, which includes:
    • A published code of ethics
    • Administration by an active board of directors
    • Admission requirements that specify college courses and other training, a bachelor’s or higher degree, at least three-years’ experience in specified fields, and a qualification exam.

• An Initial training certificate and ID card (if applicable) for each of the following courses:
  • Asbestos Inspector
  • Asbestos Management Planner
• Any Refresher training certificates and ID cards (if applicable) for each of the following courses obtained before submission of application:
  • Asbestos Inspector
  • Asbestos Management Planner

• Verifiable work experience that includes participation in at least five Asbestos Inspections performed under the direct supervision of a licensed Management Planner, Inspector, or Individual Consultant
• 1-inch by 1-inch color photo of the applicant with a white background
• Completed DSHS “Physician’s Written Statement” signed by a doctor within the last 12 months
• An Initial Asbestos Inspector training certificate and training ID card (if applicable)
• Any Asbestos Inspector Refresher training certificates and training IDs (if applicable) obtained before submission of application

• or accreditation by NVLAP for analysis of airborne asbestos by transmission electron microscopy,
• or accreditation as an industrial hygiene laboratory by the American industrial hygiene association (AIHA) and participation in the proficiency analytical testing (PAT) program for analysis of airborne fibers by phase contrast microscopy (PCM),
• or proficiency according to standards of AIHA PAT program, which includes quarterly proficiency testing for airborne fibers by PCM and a quality assurance/ quality control program as required by the NIOSH method 7400, issue 2, August 1994,
• or Accreditation of the individual laboratory analysts through the AIHA asbestos analyst registry (AAR) and a quality assurance/ quality control program as required by NIOSH method 7400, issue 2, August 1994.
