On July 1, 2014, the New York Nonprofit Revitalization Act took effect, and there is a new series of NY nonprofit forms to be filed.

If you try to file using the old forms, your filing will be rejected, causing added cost, delay, and frustration.

Below are two you’ll find most helpful:

If you’re setting up a new nonprofit in New York, you’ll use the Certificate of Incorporation.

If you’re expanding into New York from another state, you’ll use the Application for Authority.

There are also new forms for dissolving, changing the name or purpose of your organization, or changing its registered agent. You can find them all on the NY Department of State website.
